In an All Item World, that value proposition evaporates. You walk to the "Tools" section, open a chest, and grab three of them. This removes the friction of the "looter-shooter" aspect of Terraria, leaving only the creative and combat aspects behind.
For a long time, Terraria lacked an official "Creative Mode." While Journey Mode (introduced in the 1.4 update) bridged the gap by allowing players to duplicate items after researching them, All Item Worlds remain popular because they offer total instant gratification.
